Fortis Mohali restores mobility of a wheelchair-bound man with complex knee injury through Deflexion Osteotomy

Shimla, April 28, 2023: The Department of Orthopaedics at Fortis Hospital Mohali has restored mobility in a 34-year-old disabled man who had undergone two knee surgeries earlier which were conducted in Mumbai and Gurugram, respectively.
The team of doctors led by Dr Ravi Gupta, Director of Orthopaedics (Sports Medicine), Fortis Hospital Mohali, treated the Patient’s unstable knee and corrected the defect through Deflexion Osteotomy and Revision ACL surgery recently.
The Patient was in extreme pain after sustaining a ligament injury in his left knee last year. He was operated upon in Mumbai for an Anterior Cruciate Ligament (ACL) surgery. Following this, he subsequently underwent another ACL revision surgery in Gurugram. However, the Patient’s knee remained unstable as the surgery did not yield the desired results. After six months of rehabilitation, the Patient’s knee continued to remain unstable after which he finally approached Dr Gupta.
Being a complex case due to the two earlier surgeries, Dr Gupta conducted a thorough medical evaluation which revealed that the Patient had a graft rupture of the ACL. His condition was diagnosed as High Posterior Tibial Slope (PTS) of 19 degrees – which was a cause of failure of earlier surgery.
The Posterior Tibial Slope (PTS) is the slope of the upper surface of the leg bone (tibia) inclined towards the back of the knee. On this surface of the leg bone, the lower end of the thigh bone (femur) joins it to form the knee joint. If this slope is more, the thigh bone moves backwards, causing high stress on the ACL graft leading to graft failure. In addition, the MRI also revealed that the Patient also had suboptimal placement of the ACL graft in the previous surgery.
The team of doctors led by Dr Gupta conducted the complex surgery wherein the Tibial Slope was corrected through Deflexion Osteotomy and this procedure was combined with Revision ACL surgery – both the surgeries in one stage. This was done by using a new graft taken from the patella (knee cap) and upper end of the tibia along with patellar ligament.
The surgery was successful and the Patient started walking on the next day of the procedure.
Following aggressive rehabilitation and protocol designed by Dr Gupta, the Patient was able to gain full movement and flexibility. Post two months, he is able to walk normally and has resumed his daily activities.
Shedding light on the case, Dr Gupta, said, “The Patient had undergone two failed surgeries and this had badly affected his mobility. The defect was corrected through Deflexion Osteotomy and Revision ACL surgery. He has resumed his daily activities and is able to walk with ease now.”
